Malignant Schwanoma Associated with von Recklinghausen¢¥s Disease

Abstract
The histogenesis of malignant schwanoma is still controversial. Presently, it is considered to originate either from schwann or perineural cells. Authors experienced a case of malignant schwanoma associated with von Recklinghausen¢¥s disease in a 41 year old male patient.
He was admitted to the Dept. of General surgery of Sung-Sim Hospital complaining of painless masses in the left lower back and buttock of 20 years duration. One of them was excised 20 years ago. Physical examination represented several brownish nodules on the anterior chest. Laboratory findings including CBC, urinalysis and blood chemistry were within normal limits. Chest PA and ECG were unremarkable. Lumber AP & lateral views showed adult-fist sized soft mass without bone involvement of left lower vertebra. Operation was done under the clinical impression of neurofibroma. Operative findings revealed relatively ill-defined multilobular masses at the above site. Therefore radical excision was done. The submitted specimen for frozen was a skin with attached subcutaneous mass, measuring 10 x 7 x 4.5 cm in dimension. Cut surface showed homogenously yellowish white discoloration and focal areas of cystic change. Histologically hypercellular spindle cells with mitotic figures were observed. Therefore, spindle cell sarcoma was reported. Also submitted was a tumor mass, measuring 17 x 7.2 x 2 cm. On serial section cut surface was multilobulated, showing focal areas of tumor necrosis End cystic change in the yellowish white background.
Histologically tumors showed hypercellular spindle cells, arranged in interfasciculating pattern. The individual tumor cells had hyperchromatic nuclei and frequent mitotic figures. Focally scattered are areas of peritheliomatous arrangement of tumor cells, myxoid change
